Client Intake Form
The first step to prepare for your session is to complete the Client Intake Form. This form is necessary to give me the information I need to prepare for your session. It must be completed and submitted at least two days prior to your appointment.
What to know about your hypnosis session:
- There is nothing very mysterious about hypnosis. It is very similar to any guided meditation or visualization exercise that you have ever experienced. The difference is that you will have me with you every step of the way and in real time to help guide you to wherever you need to go to receive healing and inner guidance. This will be different for every person and for every session.
- The relaxation and focus that comes with hypnosis allows your conscious mind to move into the background so that you can access your subconscious mind with all of the memories that it contains. It also gives you access to your Spirit Self who will be very important in determining where you can find healing most effectively.
- All healing comes from within you! I am not a healer–I am better described as a guide or facilitator. I am here to help you relax into the altered state of consciousness that is hypnosis, guide you towards your own inner healing and wisdom, and give you the completely non-judgmental space you need to integrate that healing.
- Even though you will go into an altered state of consciousness, you will still be conscious and in full control of all that happens to you. Nothing can ever happen without your consent, and you will always be able to communicate with me about any concerns that come up or even terminate the session at will if you choose.
- Please always feel that you can advocate for yourself at any time during the session. You can ask questions that come to you, or interrupt if something is grabbing your attention. For example, if your doorbell rings or you find you need to go to the bathroom, it is better to speak up, deal with the situation, and then come back into hypnosis then to have the distraction constantly in the back of your awareness. It will take your focus away from what is being accomplished, so better to get it out of the way instead. You can always easily get back into hypnosis.
- This is a big one! When in hypnosis, always trust whatever first comes to your awareness. It may seem silly or even impossible, but it is what you need to experience for healing or you would not be experiencing it! You could find yourself back in a memory from childhood, or living the life of an ancient soldier, or even swimming in an underwater garden. Some clients simply experience a color or a smell, but whatever comes up always leads us to the healing that you need. So trust it!
- Some experiences can be troubling–even memories of trauma. However, you do not need to re-live the trauma in order to heal it. Hypnosis allows me to guide you through the experience as if you were watching it happen like a movie. You will be able to get the knowledge and healing wisdom you need from the experience without incurring the trauma again.
- You will easily be able to speak and answer questions while in hypnosis. You can even allow your Spirit Self to use your vocal cords to give you wisdom and healing directly. You will be free to ask questions and tell me all that you are experiencing. However, you will also always have the awareness to decide if there is something that you don’t want to share. Our goal is your healing and wholeness. Telling me what you are experiencing helps me guide you, and my priority is always of non-judgmental acceptance, however you don’t need to tell me everything if you feel it is not appropriate for you.
Preparation Tips:
Here are some suggestions to help you have the most relaxing, enjoyable, and healing session possible:
- A few days before your appointment, please download Zoom if you don’t already have it on your device. It is also a good idea to do a test meeting with a friend if you are not familiar with using Zoom.
- Arrange for a quiet, private place where you can be comfortable for 2-3 hours. It is also best if you can locate your computer in a place so that I can still see you. Some good options are your bedroom with the computer on the bed beside you, on a couch with the computer on your coffee table, or possibly in a recliner with the computer on a table next to you. I even had one client use a really large bean bag chair!
- Have whatever you need to relax comfortably. It is best to lay on your back without any arms or legs crossed, so a pillow for your head and maybe even for behind your knees is wonderful. Also, your body temperature will lower during hypnosis, so a cozy blanket is also comforting. Please feel free to wear loose clothing suitable for laying down to relax.
- For a truly magical hypnosis experience, it is best to use headphones with a microphone. They don’t have to be expensive, but there is just something wonderous about hearing my voice relaxing you while seeming to come from inside your own head! However, please test whatever listening and speaking tech you choose to use ahead of time so you have no worries during the session.

- Please try to have a light meal before the session–being too full or too empty can be distracting. Also, it is best not to have any caffeine before the session. Caffeine excites you, and we want you calm and relaxed!
- I can record the Zoom session if you would like to listen to it in the future, so give that possibility some thought, and let me know what you decide before we get started.
- You may want to take a trip to the restroom before your appointment, however I will ask again just before we start hypnosis. There is less chance of interruption that way.
- Some clients like to have special incense burning or other comfortable scents nearby or crystals for healing, love, or transformation. Feel free to include anything like that, however I would suggest not using anything with an open flame which could have you worrying about it in the back of your mind.
- As these sessions are 2-3 hours long, it is nice to have something to drink available especially for after you come out of hypnosis. Good quality filtered, spring or mineral water is best, but whatever you prefer is fine.
- Between now and the time of your appointment, you may have questions. If they need to be answered before your session (i.e. for scheduling, Zoom use, etc.), please feel free to e-mail me. Otherwise, start a written list so you can be sure to ask them during our conversation before hypnosis starts.
- Speaking of our pre-hypnosis conversation, I will be asking you for more in-depth information about the challenge you are looking to overcome or the healing you need. So, here are some questions to consider before your session appointment:
- When did this issue start?
- When did it become a problematic to the point you started looking for how to rectify or heal it?
- What is your earliest memory related to this issue?
- What are the most recent symptoms or feelings this issue brings up?
- What patterns have you noticed regarding this issue?
- How often do they occur?
- What makes is better? Or worse?
- This may sound odd, but are there any secondary benefits to this issue? For example, if you have migraine headaches, do they sometimes come in handy as a good excuse to not attend an event you weren’t looking forward to.
